This Position Will: Provides residential, home and community-based service coordination, therapy, case management, crisis intervention and prevention services to youth and families in order to reunify, preserve, and/or stabilize families and divert reentry into residential services. Assists youth and their families in navigating the transition from residential treatment or intensive services back into their home environments or lower level of care setting. Coordinates contact between clients and outside agencies, acts as liaison between assigned clients, the clients’ families, guardians, funding sources, and referral sources. Prepares required reports and ensures all relevant parties are informed of clients’ needs, goals, and treatment progress on a timely basis. Communicates and collaborates with school personnel, psychiatrists, nursing, and other individuals involved in the treatment of clients. Acts as a key member of the treatment team to provide comprehensive care to assist with comprehensive discharge planning and follow up care post residential discharge.

The Ideal Candidate Will Possess:

REQUIRED:

  1. Bachelor’s degree in psychology, Social Work, Counseling, or related human services field.
  2. Excellent written and verbal communication skills
  3. Valid driver’s license with good driving record
  4. First Aid and CPR certified within first 90 days
  5. Working knowledge of word processing software

Salary:

$50,336 – $65,437
